Information on possible vaccination clinic at Canajoharie CSD

The Canajoharie Central School District is possibly partnering with Montgomery County Public Health to host a COVID-19 vaccination clinic on campus. We would like to provide this opportunity to our community to increase vaccine availability.

In order for the district to have a vaccination clinic on site, we need to know how many people in our school community would like to be vaccinated. Our ability to hold a clinic is dependent on the number of people who are interested. Please complete the following google form to indicate how many people in your household are interested in receiving the vaccine. Survey responses will be shared with the Montgomery County Department of Public Health.

Any person 16 years of age and older will be eligible to receive a Pfizer vaccine at this clinic. Anyone in your household who is a student will need to be accompanied by a parent/guardian.

To be fully vaccinated, you must receive two shots of the Pfizer vaccine. The Canajoharie Central School District would hold two clinics on campus. Anyone who receives an appointment at the first clinic will be scheduled to receive their second shot three weeks later. We are trying to book April 20 and May 11 from 4-7 pm.

Please fill out the form using the link below by Wednesday, April 14 if interested.
